This is now the official PPL paper timesheet for CDPAP Personal Assistants to complete if they cannot access or use PPL's timekeeping app. The form says it is for temporary use thru 4/26/25. After that date they will need an exception from PPL not to use the Time4Care app. This form has an email address, mailing address and fax number to submit the completed timesheet, which must be signed by the consumer.
